To protect your child in an emergency, support their health needs, and maintain clear communication throughout the year, we need you to complete our 2025-2026 Annual Notification forms. These forms collect critical updates—emergency contacts, medical needs and allergies, transportation details, and permissions for technology and school activities. Completing them helps us respond quickly in an emergency, provide the proper supports, and keep every student safe, supported, and connected.

Completing these forms is more than just paperwork—it’s one of the most important ways families partner with Lamoille North Supervisory Union to ensure student safety and success.

You can complete the Annual Notifications in your Infinite Campus Parent Portal by clicking More > Online Registration > Start next to Annual Update Forms.
